Social Service Assistant

Provide assistance to residents and family members throughout the assessment and care planning process as well as transitional and discharge planning activities. Excellent communication skills required. College degree in social services or related field and MSW/LSW/LCSW desired. Experience in health care/nursing home and MDS 3.0 preferred. BA/BS required. Rehab Aide

- Hours are 8:00 am to 4:30 pm Saturday and Sunday
- This is an hourly position
- Candidates must be a Certified Nurse’s Aide & Rehab Certified
